Health Management Programs

We want you to be at your healthiest. So, we offer a range of Health Management Programs at no additional cost to eligible members. 

These programs are delivered by qualified health professionals and are tailored to your needs - whether to help get you in shape, keep you out of hospital, improve your physical and mental wellbeing or to aid a quicker recovery after you've been in hospital. Check out our range of programs below.

Honeysuckle Health Programs 

We have a partnership with Honeysuckle Health to provide our members with access to additional professional health programs at no extra cost:

Hospital Support Program 

This is a phone-based program giving eligible members access to an experienced registered nurse to help navigate the health system and understand your options before, during and after a hospital stay. Designed to accompany the usual care provided by your specialist or GP, the program may include: 

  • Personalised support over the phone  

  • A health status assessment  

  • Clinical decision-making support  

  • Support to close any knowledge or treatment gaps  

  • A Care Plan tailored to your health goals and priorities  

  • Access to an experienced nurse to address chronic conditions and follow up care needs 

This program is not available for pregnancy-related admissions. 

Mental Health Hospital Support Program 

This is a phone-based program giving eligible members access to an experienced mental healthcare nurse to provide support before, during and after an overnight mental health admission for depressive and anxiety disorders. Designed to accompany the usual care provided by your psychologist, psychiatrist or GP, the program may include: 

  • Personalised support via phone  

  • A health status assessment  

  • Clinical decision-making support  

  • Support to close any knowledge or treatment gaps  

  • A Care Plan tailored to your health goals and priorities 

  • Access to an experienced mental healthcare nurse to address chronic conditions and follow up care needs 

Care Support Program 

This program supports eligible members with chronic conditions and a history of hospital admissions. It is a phone-based program for eligible members who can benefit from health education and care coordination. The program provides access to an experienced registered nurse to help you better understand your chronic condition and achieve your health goals. Although program length varies according to individual needs, most members can expect to participate for 4 to 6 months. Designed to integrate with the usual care provided by your specialist or GP, the program may include: 

  • Personalised support via phone  

  • Education and health coaching  

  • Support to close any knowledge or treatment gaps  

  • Tailored Care Plan  

  • Medical care coordination needs  

  • Carer support 

Members will be screened to ensure they can fully benefit from the program - we can organise for our program partner to contact you to confirm clinical eligibility. 

Mental Health Care Support Program 

This program supports eligible members who are feeling overwhelmed, stressed, anxious or depressed, and who have a history of overnight hospital admissions for mental health. It is a phone-based program giving you access to an experienced mental healthcare nurse to help you better understand and manage your mental and emotional health. Although program length varies according to individual needs, most members can expect to participate for 4 to 8 months. Designed to accompany the usual care provided by your psychologist, psychiatrist or GP, the program may include: 

  • Personalised support via phone  

  • Education and health coaching  

  • Support to close any knowledge or treatment gaps  

  • Tailored Care Plan  

  • Medical care coordination needs  

  • Help connecting to community services and resources  

  • Carer support 

Members will be screened to ensure they can fully benefit from the program - we can organise for our program partner to contact you to confirm clinical eligibility. 

Cardihab

Cardihab is cardiac rehabilitation via a smartphone app, so it is accessible anywhere, anytime. Designed to follow the Australian Heart Foundation and Australian Cardiac Rehabilitation Association guidelines, Cardihab includes one-on-one mentoring from a registered nurse who will support you in your recovery, provide you with a personalised care plan and answer questions you might have throughout the program.
Questions as simple as “am I allowed to go for a walk?” or “can I have a glass of red with dinner tonight?” can be answered straight away, providing you with peace of mind that you’re on the right path to recovery.

The program duration ranges from 6 weeks or 6 months depending on your preferences and how it fits in with your lifestyle. Cardihab is designed to complement the care provided by your GP or specialist.

COACH

The COACH program is a telephone-based health management program designed to assist those who have been diagnosed with or at high risk of coronary heart disease, type-2 diabetes, or chronic obstructive pulmonary disease.

The program includes:

  • Over the phone coaching at a time that suits you

  • Practical nutrition, fitness, and lifestyle advice

  • Support to help achieve our own health goals

  • Support to know the key questions to ask your doctor

  • Education on reaching and maintaining your optimum level of health and fitness

Other Health Programs 

 

Healthy Weight For Life™  

This is a specialised knee and hip arthritis management program that has been designed to help eligible members better manage joint pain and get back to living life - all in the comfort of your own home. It includes over $800 worth of meal replacement products, personalised service and online support. The program may include:  

  • A portion control eating plan (including KicStartTM VLCD meal replacements)  

  • Activity plan and physiotherapist-developed strength, balance and mobility exercises  

  • Personalised online symptom, progress and satisfaction tracking (phone and mail alternatives also available)  

  • Two-way personal motivation, support and advice via phone, SMS, email, message board and mail  

Rehab at Home* 

An evidence-based program, Rehab at Home provides intensive physiotherapy and in the comfort of your own home following your total hip or knee replacement, if eligible. The Rehab at Home program aims to get you mobile as soon as possible. The program may include: 

  • Hands-on manual treatment including joint mobilisation and evidence-based therapies to help speed up your recovery 

  • A supervised exercise program to ensure you know exactly what you need to do to 

  • Work on building optimum knee strength and function needed for everyday activities 

  • Advice on aids and appliances to assist with activities of daily living

Silvercloud Health

Silvercloud Health is a digital mental health solution designed to support and promote positive behaviour change and mental wellness for members. It provides access to a comprehensive range of therapeutic and wellness-oriented programs and access to a mental health supporter on request. Silvercloud Health empowers you to take proactive steps to manage stress, low mood, anxiety, depression and to improve sleep. Targeted programs include:

  • Comorbid long-term health conditions

  • Student Health

Limber Digital Exercise Therapy

Limber is an at-home exercise therapy app designed to support a range of musculoskeletal conditions including low back, shoulder, hip, neck, and knee joints. Developed by health professionals, Limber gives members access to a home exercise program and tailored to meet their needs.
The program includes:

  • Easy access via mobile app

  • An initial assessment to understand your health needs

  • Tailored home exercise program based on your health goals and priorities

  • Video demonstrations

  • Ongoing assessments

  • Access to an exercise physiologist through in-app messaging

Hello Sunday Morning

Hello Sunday Morning's online Alcohol and Wellbeing self-assessment tool will help you to better understand your drinking habits and take care of your health. By taking the five-minute confidential assessment, you can get a personalised snapshot of your drinking behaviours and be directed to clinically-backed health and wellbeing information, advice, and assistance to suit your individual circumstances. This includes Hello Sunday Morning’s Daybreak app, which provides free access to 24/7 digital support and an online support community of like-minded people looking to change their relationship with alcohol.
